The MĪ Care Survey and Plan are grounded in the renowned Theory of Multiple Intelligences, developed by Harvard cognitive scientist Dr. Howard Gardner. This foundation is enriched by the practical expertise of CFHDC co-founder Matthew Call, who has applied Gardner’s principles while working with hundreds of individuals living with dementia.

What Is the MĪ Care Survey and Plan?

The MĪ Care Survey is an evidence-based assessment tool designed to identify the preserved strengths—known as intelligences—of individuals with dementia. Unlike traditional assessments that highlight deficits, this survey focuses on what remains intact. The resulting personalized care Plan is AI-generated, combining each person’s unique abilities with their personal history and preferences to support more meaningful engagement and care.

How It Works

The survey is designed to be completed by the person who knows the individual best—often a caregiver or close family member.

  • It takes about an hour to complete.
  • The questions draw on eight of Gardner’s intelligences, including:
  • Logical-Mathematical
  • Linguistic-Verbal
  • Interpersonal

  • Intrapersonal

  • Visual/ Spatial
  • Naturalistic
  • Musical

  • Bodily-Kinesthetic
  • It also includes prompts to gather important personal and contextual information.

Once submitted, AI instantly analyzes the responses to produce a personalized, easy-to-follow, strength-based care plan—available within seconds.
